The corrected pages in their entirety (pages 3550-3552) are published on Blood Online as a supplement to the article.

On page 3550, paragraph 2, citation 33 should be 34 and citation 34 should be 35.  On page 3551, citation 35 should be 41, citation 36 should be 42, and citation 37 should be 43.
